El cuero cabelludo puede ser propenso a una mala cicatrización porque los cierres de las heridas quirúrgicas suelen estar sometidos a una gran tensión en esta zona, y el cuero cabelludo es propenso a sufrir daños actínicos difusos. La silicona puede tener propiedades ideales para estimular la cicatrización de las heridas del cuero cabelludo. Nos encontramos con cuatro casos de heridas del cuero cabelludo que no cicatrizaban, todas ellas caracterizadas por un tejido de granulación exuberante y una falta de migración epidérmica presente durante al menos 2 meses después de un acontecimiento desencadenante. En cada uno de los pacientes, la aplicación de un gel a base de silicona formadora de película directamente sobre la herida como monoterapia, sin un apósito secundario, se asoció a una mejor cicatrización de la herida.

La dermatosis pustulosa erosiva (EPD) es una afección cutánea inflamatoria poco frecuente de etiología desconocida. Suele aparecer en el cuero cabelludo de mujeres de edad avanzada tras traumatismos locales, como cirugía, radioterapia, crioterapia, infecciones por varicela zóster y lesiones actínicas. Inicialmente, la EPD se caracteriza clínicamente por erosiones, grandes costras y pústulas que persisten y resisten el tratamiento durante meses o incluso años. El diagnóstico de la EPD suele basarse en los hallazgos clínicos, ya que la histopatología normalmente muestra cambios inespecíficos con infiltrados linfoplasmocitarios dérmicos ocasionales, atrofia y pérdida de folículos pilosos.

El aumento del número de procedimientos cosméticos quirúrgicos y no quirúrgicos ha dado lugar a una mayor demanda para conseguir cicatrices estéticamente aceptables. Los apósitos de gel de silicona (SG) se evaluaron en estos casos siguiendo los principios del cuidado de heridas y minimizando también la formación de cicatrices anómalas. Una solución recientemente desarrollada en el cuidado de heridas en forma de SG ha demostrado ser un tratamiento altamente eficaz para una serie de 4 casos clínicamente desafiantes presentados en este artículo: cicatrización posterior a un tratamiento con láser, heridas del cuero cabelludo que no cicatrizan, queilitis eccematosa xerótica recidivante crónica y el tratamiento de cicatrices causadas por quemaduras de tercer grado. Se aplicó un SG estándar para mejorar el resultado cicatricial de las quemaduras graves de un niño pequeño. Los geles de silicona ofrecen excelentes resultados clínicos en estos 4 casos. En términos de cuidado de heridas y tratamiento de cicatrices, proporcionan una forma de aplicación cómoda y fácil de usar y aumentan la comodidad y el cumplimiento por parte del paciente. Para continuar con estos resultados, es necesario llevar a cabo más estudios, pero por el momento existen pruebas sólidas de que los geles de silicona presentan propiedades beneficiosas para el tratamiento de heridas y la prevención de cicatrices.
